General Employment Information

Job Coach: The Job Coach will support clients who are employed, participate in productive or skill development activities or who are in the process of employment planning. The role of the Job Coach will range from assessing client interests and skills to providing ongoing support and evaluation of each employment situation. They will train clients in specific work-related areas and obtain opportunities for them to observe or directly experience various employment options. The Job Coach will also support on the job facilitation of relationships with work colleagues and provide employment follow-up and /or transition planning.

Team Leader: The Team Leader will train, supervise and support Primary Care Workers to work with clients within their projects of responsibility. The Team Leader will also promote the overall personal wellness of each client, participate in the development and implementation of individual plans, train and support client independence and integration, provide personal care support and ensure that a positive and safe home environment is maintauined.

Community Networker: The Community Networker will provide ongoing support to clients who can be employed for 24 to 27 hours a week (Sun-Sat), and to clients who do meaningful and fulfilling activities within their community. They will assess the client's interests and skills and connect them with volunteering and/or community groups. The Community Networker will also match clients to financial contract positions and provide the clients with job skill developments such as signing in, appropriate appearance, job interactions and proper quality work.

Primary Care: The Primary Care Worker will promote the overall personal wellness of each client, participate in the implementation of individual plans, train and support client independence and integration, provide personal care support and ensure that a positive and safe home environment is maintained.
